If you have a Mac, it is easily done from within the browser, without add-ons.
Tell me if you have, and I’ll specify.

you can use keepvid http://keepvid.com you copy the youtube video url into the box, press the button, and it loads a page with download options. most of the time it will only give you a .flv option which you’ll need a flv player for. i recommend wimpy fly player (simple program, no install needed, just run the .exe file) keevid works on other sites besides youtube, like google video.

You can play .flv also with VLC, and with QuickTime (with QT only if you have installed “Perian”).

VLC has an option to change into many formats, like .mpg. Strangely enough, that .mpg may not be playable in QuickTime until you have taken it into a burner application like Toast, and “exported” it.

If you have QuickTime with Perian, you can play te .flv directly or export it into lots of formats already from there, like for saving on your iPhone or iPod Touch.

Also, if you download realplayer, whenever youre on youtobe and watching a video, a thing will pop up above the vid, if you put your mouse on it, and if you click it (it says, Download this video to RealPlayer), it will download it.
