Formal means just that. Formal wear REQUIRED. Tuxes for guys. Ankle length dresses for the ladies. Flask of bourbon optional.

Corsage and yes you do. Find out ahead of time the COLOR of the dress. The corsage should compliment, not match her dress. Her mom could probably help in getting you that info. You might even ask her to go with you when you pick out your tux. Rent early enough so you have the widest possible selection. That way your not stuck with whatever they have left.
