Where can I find a health insurance plan with these specifications in CA, Los Angeles?

Plan: HMO or PPO

Deductible: $0

Coinsurance: 0%

Office visit: $0-$50

Monthly payment:$0-$150 for sure no more then $200!

If any of them has Dental & Vision that would be great but not required by me.

I want the plan to go into effect ASAP. so that would be around 4-1-2009 I assume.
