How do I write a two weeks notice letter?

I’m quitting my current job, (I’m 17 and still in my parent’s care and they support this decision because no one relys on me for support etc.) but I’m not sure how to write it. I’m quitting because I hate the work, hate my boss, and the schedual is so stressful. But I can tell her that can I? I need to stay on good terms with her if I want to use her as a reference. Help!

the letters nick posted are great. when resigning, the last thing you want to do is say something bad about them to them. You never know when you might need them as a reference.
