This list doesn’t have to be set in stone, things can change, but it does give you good direction. Only put down the really important things (i.e. Marriage, living in New York, etc.) once you have a solid list of a few important things focus on how to get them. for example, if you want to move to New York and have a high paying job, you’ll probably want to go to college in that area. If you want to go to college you’ll have to get good grades and do a lot of extra curriculars right now. Knowing that the things your doing now are giving you want you really want later will help motivate you to keep trying. If you focus more you should be able to stick with it and feel better about yourself. Good luck!

Just a week ago I saw a 15 year old jamming with a jazz band in a high class bistro here in my city (and it’s not a big city). The kid was really good! Get out there and find the opportunities where you are. You don’t have to do some huge change.
